Responsible for maintaining up-to-date records for each child while performing service related tasks related to on-going and immediate health needs. Coordinate with Health Managers and Head Start staff, to have all children screened for hearing and vision. Initiate a referral for all children who are in abnormal range for hearing and vision to appropriate medical entity in accordance with policies and procedures. Maintain a health record on each child which includes all required information such as physical examination, dental examination, medical background and sensory screenings. Maintain and monitor a comprehensive record keeping system. Maintain and document on-going contact with local programs through phone and site visits. Attend and participate in required community activities, meetings, and trainings on and off site. Perform other related duties as assigned from time to time.
Minimum of a high school diploma or equivalent and up to one year of relevant experience required. Knowledge of neighborhoods served by the program and relevant city-wide services required. Strong written and verbal communication as well as interpersonal skills. Bilingual skills and the ability to translate as necessary preferred. Frequent traveling for program needs and on and off-site trainings. Must be able to work independently without intensive supervision as well as a member of an interdisciplinary team. Must be able to work sensitively and effectively with individuals of diverse educational, socio-economic and cultural backgrounds.
